If you are authorized to represent your agency and need a password to gain access to the Agency Fee Imposition Report application, you can contact IOC Security Administration Unit at (217) 782-5185. You will be required to give your name, contact phone number, and agency. Once you have been authorized, someone will contact you with your agency number, user id, and password.
